On November 30, 2012 , you were issued an Employee Violation Notice wherein you were asked to explain the loss of Twenty Two Thousand Pesos (PhP 22,000.00) under your custody as cashier. In your written explanation you admitted that you were negligent in handling and safekeeping of the said amount as you admitted that you placed it on top of the credit card terminal knowing very well that it was an unsecured and an open area and leaving it there for over an hour.

Being the cashier, it was your responsibility to ensure that company funds are handled properly to avoid loss. As per company policy, employees shall be held accountable for any loss due to negligence or carelessness. Employees with custodial responsibilities should safeguard assigned property.

Please be informed that the above-mentioned negligent acts/omission committed by you, constitute gross violation of company policy, more specifically under E5 of the company policies .

In addition please be informed that because of your gross negligence in the performance of your duties and responsibilities, the company suffered financial loss.

Likewise , the above-mentioned acts or omissions committed by you are grounds for termination of under Art 282 of the Labor Code for loss of trust and confidence.

You are hereby directed to appear before the Investigating Committee composed of _______________________________________ , ____________________________ , and _____________________________ on ____________________ at ____________ , for the formal investigation of the charges against you for gross violation company rules and regulations to explain why no disciplinary action including dismissal should be imposed against you for gross violation of company policies.
