INFORMED CONSENT

I ______________ am being asked to participate in a research entitled “The Relationship of Having Absentee Parents to the Study Habits of Students of DLSHSI”. This research is being conducted under the supervision of Ms. Daisy Doctor, RN, RM, MAN

The investigator hopes to learn the relationship of having absentee Parents to the study habits of students of DLSHSI from the research.

While participating in this study, I will answer the questionnaire that will be given by the researchers. The questionnaire has two parts, the first part pertains to my demographic date and the second part is an assessment of my time management, motivation to study and my study techniques.

Ms. Coleen I. De Torres, Mr. Rodman T. Dela Santa, Ms. Lores Pearl E. Loredo, and Ms. Liana Monique P. San Lorenzo have explained the nature of this study to me. I understand that the anticipated benefits of my participation will be for the awareness regarding my study habits, enabling me to reflect on them.

The researcher will make every effort to safeguard the confidentiality of the information that I will provide. Any information obtained from this study that can be identified with me will remain confidential and will not be given to anyone without my permission.
