Fitness Assessment and Plan for an Older Adult
Par-Q Rating
After completing the Par-Q assessment (Form 3.1) a doctor’s permission is not necessary to
begin an exercise program. The client has no major health conditions or any health issues or risk
factors that would prevent her to start light to moderate exercise routine.
Health Status Questionnaire
Since Patti is moderately active, she is able to complete the normal physical fitness tests. After
completing the Health Status questionnaire (Form 3.2), a doctor’s permission was not
required. Currently her health status is good. There are no risk factors or any health conditions
I therefore, decided to give her a ‘go ahead’ to start an exercise program.
General Medical History
Age: 61 Height : 5’1” Weight 135 lbs
No major medical history or issues in regards to general overall health.
Assessment Results on testing
1. CRF: One mile walk finished in 20 minutes
To get the full benefit of her cardio workout, she must consider walking for 20-30 minutes at 60-
90% of your maximum heart rate.
To determine her max heart rate, I subtracted the age from 220. To determine the target zone
multiply that number by 60% and 90%. So, being that she is 61 yrs old (220-61=159), multiply
159 by 60% (159 X .60=95). This means she would need to keep her heart rate at 95 beats per
minute for at least 20 minutes. The best way to improve her cardiovascular fitness is to gradually
increase to the higher end of the range. To get the full benefit of her cardio workout she must
consider walking for 20-30 minutes at 60-90% of your maximum heart rate.
2. Endurance: She performed the push up test for the endurance assessment. She was able
to do 20 counts of pushups. Rating: Moderate. She would like her to improve the
