Main causes of injury/potential causation factors
Fatigue
• Not following procedures and policies
• Cultural factors – Language barriers, Literacy problems. Eg. Not understanding any safety instructions given clearly
• Fitness for health and work. Eg. Lack of sleep
• Lack of experience
• Poor supervision
• No team work
• Not being proactive – Lack of initiative, not reporting incidents and hazards
• Bad communication with others
