I was asked to create a continuity binder for processing EPRs. This is more or less what I created. Hope this helps someone do less work on theirs.

FOREWORD
This guide is aimed at helping you write effective performance reports. Effective performance reports are timely and accurately reflect the performance of the ratee. Timely and accurate reports benefit the Air Force by moving the right people up through the ranks.
This guide was not written to stand alone; it does not replace the governing Air Force Instructions (AFIs) or higher headquarters guidance. In fact, if it has been a while since you read the AFIs, stop and read them before you proceed with this guide.
The Nuclear Weapons Center Performance Report Writing Guide is a concise reminder of points to remember when writing a performance report. It points out areas where mistakes commonly occur. This guide should help prevent the need to correct a report. The goal: timely and accurate reports written right the first time, without the need for correction.
