Leadership Budget Change Project

The mission of Quality Home Health Services is “keeping with the three-fold ministry of Christ - Healing, Preaching and Teaching - QHHS is committed to providing quality health care” The vision is “We will be the provider of choice by transforming the delivery of health care through partnering with patients, families, physicians, care providers, employers and payers; and by offering safe, integrated, patient-focused, high quality, innovative cost-effective care”.

This project is to propose a budget for improvement in communication and team work for Quality Home Health Services. This is a new home health agency so there is not money available for this project. The budget resources for this project will be people and time.

Effective clinical practice involves many instances where information must be accurately communicated. Team collaboration is essential. When health care professionals are not communicating effectively, patient safety is at risk (Sullivan, 2013). Lack of communication creates situations where medical errors can occur. Collaboration between physicians, nurses, and other health care professionals increases team members’ awareness of each other’s knowledge and skills.

My first step will be to organize a meeting to have an open discussion so everyone can voice any issues or concerns they have. The next step is to review the information and integrate it with other goals and objectives that are needed to improve team work and communication. Once the data is gathered our administrator can then review it for approval. After a plan is finalized, the last step is to meet with all the staff to present the projects with goals and time frame of expectations.
