Failure Mode and Effects Analysis (FMEA) was one of the first systematic techniques for failure analysis. It was developed by reliability engineers in the 1950s to study problems that might arise from malfunctions of military systems. There are many types of software available for this analysis. In this case we can use Excel program for this analysis. We create an Excel program for analysis the failure mode and the effect of the failure. We also identify the necessary action and after the taking of necessity action we evaluate the failure mode with the new assumption data. So we will get some failure could be recovered. We use three colors for identify the failure as follows: Green: Indicates that we are in safe stage and not so much failure chances. Yellow: Indicate that the rank of failure is moderate. Orange: Indicate that the rank of failure is high.

Under this assumption I found as potential failure is “No call back”, so the effect of the failure could be “Customer angry”.
