REPORT ON THE PREVENTION OF FRAUD, WAIST AND ABUSE IN THE OFFICE
1. INTRODUCTION
On Monday 1 June, 2014 you asked me to investigate the increase of fraud, waste and abuse in the office and come up with any recommendations that I can implement and improve our process. My report was to be submitted to you no later than 17 June, 2014.
2. FINDINGS
A) After a thorough investigation throughout the department, I was able to identify some concerns that I noticed across the floor which contributes to this vast amount of Fraud, Waste and Abuse. With proper attention, I think the department should be able to reduce its cost.
1) A number of associates are ordering equipment online without going to the supervisor first for cost.
2) There has been an extensive use of Fed-Ex envelopes being used to send checks out instead of a regular envelope.
3) A number of employees also have been ordering archived boxes online requesting next day delivery instead of 3-5 days delivery time.
I analyzed the cost of the departments use of Fed-ex and online ordering, comparing this year’s second quarter with the first, and this year’s usage to date compared to last year.
The Fed-Ex bill for the second quarter of this year is 25% higher than the first quarter (Jan-March: $1,050.00: April-June $800.00). With the increase in cost, the department’s bill for this year to date compared to the same period last year is some 30% higher-$1,365.00 compared with $1,205.00 last year. The increase in cost has nothing to do with business increasing but the lack of supervision.
3. CONCLUSIONS AND RECOMMENDATIONS
The investigation that I have made justifies the concerns that you expressed about FRAUD, WASTE AND ABUSE, which is driving the cost of the department to increase. The increase in using Fed-Ex...
