Program Evaluation
The program will be evaluated through surveys and communication with the patients who are receiving services through the program. The surveys will allow the administrators to make a report to deliver to the Board of Directors to make alterations to the program to make it more effective for the patients. The program evaluation will allow the program to better meet the needs of those who utilize the services of the program. The mission, goals, and objectives of the program will be put into question in the surveys to ensure the employees and volunteers are performing the services they are directed for each patient.
Evaluation Process
Collecting data through statistical information on the usage of services in the evaluation process will be synthesized through computer software. The WellSpace Mobile Care clinic will conduct quarterly reports processing data every week for the first three months the service is up and running. Data will be collected in multiple areas to include number of calls received, number of scheduled appointments, number of people that actually receive care, number of patients that are evaluated and referred out for each patient, and cost effectiveness. Comparisons to other WellSpace Mobile Care Clinics will generate feedback on service implementation and customer response. The administrator will analyze all data and report the findings to the program director for further review by the fifth of every month. If improvements are suggested the administrator will implement the improvements into policy to make necessary changes.
