SUBJECT: What to do when Your Trainee is Ready to be Upgraded to the Next Skill Level
1. When you want to recommend someone for upgrade please do the following:
a. The supervisor needs to make an annotation in the trainees AF Form 623, recommending the trainee for upgrade. (Example: Airman Jones has completed all mandatory requirements for upgrade to the 5 or 7 skill level and supervisor sign your name and date). The date should be a current date.
b. The trainee and supervisor will sign the AF Form 623a.
c. Forward the trainees AF Form 623 (individuals training record), the master task listing, and 7 level school certificate (if upgrading to 7 lvl) to me for review.
