Mr Jamal’s health report

Introduction:

This report will analyse Mr Jamal’s health problems and issues which he is facing at the moment. According to the health assessment he is suffering from high blood pressure, sleeping disorder, stress and overweight.

A care plan has been devised for Mr Jamal to improve his health and wellbeing. Short and long term targets have been set for him to meet. Different actions need to be taken to meet these targets and an evaluation will be done after set time.

Mr Jamal is suffering with high blood pressure. He is taking indapamide for his blood pressure but still unable to control it. I have recommended him to start atenolol with it, which will help to control his blood pressure immediately by reducing the pressure on his heart this will also keep the heart beat slower as well.

Mr Jamal is aware off the dangers of high blood pressure and its effects on his physical, mental and social life and every aspect has been discussed in the fact sheet to Mr Jamal. So he needs to monitor his blood pressure at home. Learning to self-monitor his blood pressure can help motivate him to do different actions to meet the targets in the care plan.

The treatment of high blood pressure comes in many forms. Mr Jamal has also been advised to change his life style and diet to keep his blood pressure lower. He has been referred to a nutritionist to get advice on a low calorie diet that should be high in potassium, magnesium and fiber to keep his blood pressure lower. Mr Jamal also needs to reduce sodium intake in his diet.

Mr Jamal’s BMI is overweight. He is 3 stones overweight according to his height and his pulse rate is higher than normal as well. He needs to reduce his weight. Carrying too much weight around his waist can put him at a higher risk of high blood pressure, a higher level of cholesterol and diabetes.
