Hypothetical Working Agreement

This is a working agreement entered into on June 20, 2010 between the therapist and Freydia Jones. Freydia is a 27-year-old mother who has been using crack cocaine for four years. Her children, ages 7 and 9, have been removed from her care by Child Protective Services and are currently living with her mother. Upon entering the drug program Freydia states she wants her children back. In setting up this working agreement the therapist will use assessment, goal setting, contracting, and evaluation. The working agreement will state the purpose and key elements of each part. The agreement will also describe the utilization of each part.

The first part of the agreement is assessment. Assessment requires gathering and analyzing information about the client. This helps the therapist to understand better why the client is seeking help and how the therapist could help her. With assessment the therapist finds the client’s name is Freydia Jones; her address is 714 Centennial Avenue, Frankfort, Kentucky, 40601; and her phone number is (502)226-1234. The assessment also states that Freydia is a single 27-year-old heterosexual female who is unemployed at the present time. She is of white-non-Hispanic descent, a Christian, who does not attend church regularly, and she graduated from high school in 2001. Miss Jones is presenting with a diagnosis of drug addiction who has been abusing crack cocaine for four years. Freydia is a very strong willed individual who has stated that she will do whatever it takes to have her children living back with her. Miss Jones comes from a supportive family including her mother, one brother, and one sister who are willing to help her in any way they can to get her back on her feet so she is able to have her children back living with her....
