“For patient engagement in care of the individual, there is good evidence that specific interventions can improve patient knowledge, self-efficacy, and some outcomes, and reductions in utilization or costs of care have been reported in some studies (Schoell 2010). This evidence leads to developing strategies for educating them regarding conditions and involving them in healthcare decision making. PCMH is a “promising” model for healthcare delivery but the engaging patients and families reinforces care for the individual, practice improvement and better health outcomes. These elements lead to better cost of care, health outcomes and patient experiences (schoell 2010).

As the optimization of patient care is an on-going process, the development of patient centered medical home promotes health care delivery for patients of all ages in an effort to achieve better health outcomes.

This model system was proposed by the American Academy of Family Physicians, American Academy of Pediatrics, American College of Physicians, and American Osteopathic Association in 2007. The model follows a system that “provides comprehensive” and continuous health care under the direction of a primary care physician. This system provides the capability of affording patients high-quality, accessible and efficient health care.” This model of quality and accessibility enhances the patient experience at the primary care level offering a stronger relationship between the patient and primary care provider.
