1. Each experiment is to be answered in the course assignment area. All questions are to be

answered, in the event any of the responses are incomplete or inadequate, they will be

returned for resubmission.

2. There is a signature sheet that is available in the course. This form must be mailed to me

once all the experiments have been completed. This form is necessary and will be

maintained for at least five (5) years. Each experiment performed must be under the

supervision of either a radiologist, MD with radiation approval, an R.T. or a C.R.T. and it

is their signature that is required.
