Purpose: Many patients will get clinical discharge patients to inform them of steps they need to take to address a health issue or issues. The purpose of this assignment is to have you critically assess a discharge order and revise it in a way to increase its effectiveness.

Procedures and tasks: Attached you will find an actual discharge instruction and accompany materials for a patient with heart failure. Your task is to specify: 1) the strengths of the discharge instructions and accompanying materials, and why you consider the topic(s) you mentioned a strength; 2) the weaknesses of the discharge instructions and accompanying materials, and why you consider the topic(s) you mentioned a weakness, and 3) propose modifications of the discharge instructions and accompanying materials and why what you are proposing would be an improvement over the current materials. For the latter, you should provide some theoretical and/or empirical foundations for the proposed modifications. Note, the proposed modifications may include interactions you have with your patient immediately before or after the provision of the discharge instructions and accompanying materials.
