Introduction to assessment of Cerebro Vascular Accident (Stroke) and Hemiplegia

Stroke was originally explained by the World Health Organization in 1978 and remains defined as ‘a clinical syndrome typified by rapidly developing signs of focal or global disturbance of cerebral functions, lasting more than 24 hours or leading to death, with no apparent cause other than of vascular origin’ (Occupational Therapy Evidence Practice in Physical Rehabilitation, 2006, p. 200)

Classification of stroke

Symptoms

* SUDDEN numbness or weakness of face, arm or leg - especially on one side of the body.

* SUDDEN confusion, trouble speaking or understanding.

* SUDDEN trouble seeing in one or both eyes.

* SUDDEN trouble walking, dizziness, loss of balance or coordination.

* SUDDEN severe headache with no known cause.(National Stroke Association, 2010)

Women may report unique stroke symptoms:

* sudden face and limb pain

* sudden hiccups

* sudden nausea

* sudden general weakness

* sudden chest pain

* sudden shortness of breath

* sudden palpitations (National Stroke Association, 2010)

Early Recognition

Sudden-onset face weakness, arm drift (e.g. if a person, when asked to raise both arms, involuntarily lets one arm drift downward) and abnormal speech are the findings most likely to lead to the correct identification of a case of stroke (Goldstein and Simel, 2005)

Proposed systems include FAST (stroke) (face, arm, speech, and time), as advocated by the Department of Health (United Kingdom) and The Stroke Association, the American Stroke Association (www.strokeassociation.org) , National Stroke Association (US www.stroke.org), the Los Angeles Prehospital Stroke Screen (LAPSS) and the Cincinnati Prehospital Stroke Scale (CPSS).
