My Leadership Smart Goal

Introduction

The SMART goal I chose for myself for leadership development is to develop a communication hand off tool that the Interventional Radiology( IR) can use to promote the safe hand off of patients received from the in-patient units at my health care organization. I chose this because I can vision a potential patient safety event occurring as a result of the current process of interdepartmental transfer of our patients. The fast pace of IR demands that our nurses are exposed to quick turnovers often cutting corners which at the present time, receiving no report from the non-critical care in-patient units. This is not only poor practice, but could result in a serious patient safety event. I’m in a newly developed clinical coordinator position therefore I feel it my responsibility to target processes that are not working in my department, or are potential safety concerns. It is my goal to provide a quick glance tool for my nurses that will include pertinent past medical history, check off list of need to know items such as NPO status, allergies, current antibiotics and pain medications and last doses taken, and any concerns the floor nurses thinks is appropriate for us to know prior to providing sedation. This plan will facilitate an improvement in the communication between our units. I will also provide education to the in-patient unit nurses on the importance of this information for us to provide safe patient care in our department. Often, nurses feel an added form to fill out is just busy work. If they know the importance, they will be more inclined to comply to our request for the information. I’m planning on trialling this for about a month, survey the nurses, and do final adjustments and finalize the form. Surveying the nurses will provide me with necessary feedback...
