DIRECTIONS:
Select ONE ACTIVITY FROM EACH of the 6 wellness dimensions to complete this semester. Each activity you choose MUST be something you have NEVER done before. Check the activity off the list. You may perform an activity not listed, but you must specify what it is in the “Other” category. Take note that the dimensions do overlap, but you must do a total of 6 different activities (1 for each of the 6 dimensions).
