MEMORANDUM FOR RECORD

SUBJECT; Conduct of Lesson , Army Correspondence

1. The purpose of this memorandum is to capture the relative information concerning the conduct of the class taught by SSG XXX on 12 July 2011 for historical record purposes.

2. In this class I have learned that when you are preparing a DA 638 you need to include a proposed presentation date for the award, as to give it adequate time to make it through all of its proper channels in a timely manner. Also that that there are required numbers of achievements required for the award being recommended. Another thing that I learned about the preparation of a DA 638 is that if you don’t save the name in block 4 when you print it, it won’t have block 4 filled in.

3. I have learned many things about preparing a DA 2823 properly, including that I have never done it properly. One of the things that I learned on how to properly fill it out was that you never sign or initial it until after you have taken the oath. And also that you must not abbreviate any of the unit information in block8. And that you must use your full legal name in block 5, you cannot abbreviate your middle name with just the first initial.

4. Preparing a Memorandum for Record is more difficult than I initially thought it would be. I learned that when you prepare the memorandum spacing and font are vital to it being done correct. Also that if the signature block is not properly centered on the page the memorandum will not be considered a valid memorandum. When preparing a Memorandum for Record you must always ensure to make the dates the same, you cannot abbreviate the date in the header and not abbreviate it when using dates in the body.
