99205 – Level 5

An initial consultation suggests a new patient rather than an established one. Multiple symptoms, which can all be considered serious for a patient of that age, along with the necessity of comprehensive history, and an examination being performed is why I chose selected this code.

• A 30-year-old patient presents complaining of flu-like symptoms characterized by unremitting cough, sinus pain, and thick nasal discharge. An examination reveals bronchitis and sinus infection. The patient is prescribed a 5-day course of Zithromax.

99214 – Level 4

Since no initial consultation or history was required for this patient it is suggested the patient is established. The symptoms the patient complained about along with the location of the symptoms would give the physician a good idea of where to start, so the examination would be detailed.

• Established patient on Lithium presents for routine blood work to monitor therapeutic levels and kidney function. A nurse reviews the results and advises the patient that tests are normal, and no change in dosage is indicated.

99212. – Level 2

The patient is established, as it states and is having routine blood work done; this suggests the patient and the staff have a routine with this patient. The history is problem-focused, as is the examination because they are testing for something specific. The diagnosis is straightforward for the same reason as well as there being no change.

• A 62-year-old diabetic female presents for check-up and dressing change of wound on left foot. An examination reveals the wound is healing. The nurse applied new dressing and patient will return for a check-up in one week.
