Dear Ms. Linda Seth,

After a thorough IT Audit of your system, we have identified key areas that need to be adjusted. It has been brought to our attention that your company’s system development processes, although effective, have a few flaws that may be decreasing the full potential of the company.

There are no official system development process and system access; which can lead to inaccuracies in the system and unauthorized changes. We recommend that a structured method be developed for the initiation of system development. Such as holding periodical meetings with IT employees and representations from non-IT departments, to receive and discuss further suggestions. Also during development of the programs, one standard programming language should be used.

There is also no segregation of duties within the IT department and this is a great concern as it creates a high probability of unauthorized changes, therefore reducing the integrity of the data and a lack of accountability among the IT employees. We recommend that you clearly define the duties and restrictions among the IT staff and monitor these onwards.

The last issue identified is the lack of proper Information documentation. Proper documentation is needed in the aid of the review and control of changes within the system. In order to reduce the effect of this problem all requests for change must be documented and a formal approval process must be implemented.
