Health History of Jonathan Poe as per Michelle Poe (Mother)

Age: 8 years old Height: 48 inches Weight: 62 lbs BP: 102/60

Since Jonathan is only 8 years old, the Par-Q does not apply to him when gaining a health history. It is designed for people 15-69 years of age. As far as the Health Status Questionnaire, we gained knowledge that he is taking 15mg/day of Ritalin LA in order to manage ADHA. There is no history of any heart issues or breathing problems. Therefore, no physician consent is necessary to administer the fitness test. He is a healthy and active boy, so there are no risk factors involved.

Fitness Assessment and Plan: Par-Q & You

According to the “Par-Q & You” questionnaire (Form 3.1), a physician’s consent is not necessary for me to participate in moderate or physical activity. I have no health conditions or risk factors that would inhibit my ability to start a vigorous exercise routine. Furthermore, I am not currently on any prescription medications.

Health Status Questionnaire

After completing the “Health Status” questionnaire (Form 3.2), the same conclusions were made in regard to physician’s consent, health conditions, and risk factors. I have been participating in moderate to vigorous physical activity about three days per week (excluding regular daily activities). For example, twenty-five minute elliptical exercise with resistance, one hour brisk walk/jog with my dog, or a two and a half mile brisk walk, and some calisthenics. I would like to see an improvement in my exercise frequency to five days per week as well as a weight loss of ten pounds.

Assessments and Results

1. CRF: One mile jog – Completed outdoors on a measured course around the neighborhood.

b. VO2max performance assessment: 49.54 ml/kg/min
