S: Patient is here today because he was riding his bicycle down a steep hill and was flipped over handle bars. Was found unconscious and covered in blood.

O: Paramedics assessed and vitals where stable. Abdomen was distended and tender when physician exanimated patient. Physical examination showed that patient hit his head rather hard and was bleeding from a cut in the back of his head.

A: Ran extensive labs came back resulting that patient has sickle cell anemia and Hodgkin’s lymphoma.

P: To be followed up with specialist for possible signs of leukemia and treatment.

S: 40 year old man, cutting the lawn, which he does for his own personal business. He was surrounded by hornets and was eventually stung on the face. A while after being stung he started to feel dizzy and tongue began to swell.

O: Patient seems to be conscious and in some pain. Having a hard time swallowing at this time do to the swelling of the tongue. Gave antihistamines for the swelling, exanimated tongue and shows a prick on the inside of the tongue.

A: prescribe antihistamines for swelling and encourage patient to becareful when out about working.

P: to be followed up by allergist to see if medication is working well and if patient might have any other allergies related to working in gardens.

S: patient came in complaining of increased thirst and frequent urination. States that her vision has become blurred and has sores on her legs that haven’t healed.

O: physical examination shows that patient is rather healthy with just a few colds a year but nothing major. Has had her appendix removed when she was 7. Is pale in the skin, eyes are protruding out of eye socket, is drinking about 1 gallon of water in an 1 hour time frame. Her sores don’t look like they are from falling or scraping herself on anything. They seem to be old sores, cleaned and put medication on them to see if they help...
