Persona Academic Success Agreement

This Personal Academic Success Contract is in effect of June 29,2011, between Lanisha Blackwell, a student at DeVry University and trusted supporters.

Whereas my academic mission is the following: After the next three years of attending DeVry University my goal is to have my Bachelors in the Technical Management program. To do that, I have intentions of passing all my classes. I’m also intended to schedule at least 40-45+ credits yearly to maintain my expectancy graduation time.

Whereas DeVry University, through its administrators, faculty, staff, and support personnel, has expertise in the areas of undergraduate education, professional development, and career services; I want to gain expertise in the areas addressed by this course.

Where, others agree to support my goal to obtain a college degree.

Now, in consideration of the above, realistic terms, we all agree as follows:

1. I appoint to DeVry University to provide the teaching and advisory services described in its enrollment agreement for the term of this Contract upon my admission and upon having met with my advisor. With this contract is a plan of study attached, which I will complete to the best of my ability, making adjustment as necessary and advised.

2. I appoint DeVry University to provide to me, by and through teaching and other services related to the day-to-day operations of my education, strategic planning, ethnical and comprehensive education practice, and financial advice related to my education, regarding course scheduling and planning with respect to my known financial, emotional, and educational abilities.

3. For the delivery of the educational and advising services listed in this success contract, I agree to pay to DeVry University all applicable tuition and fees as dictated by my course enrollment beginning at admission and continuing through the period during which all courses are taken, until I graduate.
