CONSENT AND WAIVER OF LIABILITY

HOLD HARMLESS AGREEMENT

KNOW ALL MEN BY THESE PRESENTS :

That I, _________________________________________ of legal age, Filipino, single/married, residing at _________________________________________, parent/legal guardian of _____________________________________ of age, do hereby expressed my consent over my child’s/ward’s entry within ______________________________________________________.

It is understood that the minor/ward, below eighteen (18) years of age, shall be accompanied at all times with almost supervision by the parent /legal guardian while inside the premises

It is understood that alcoholic beverages of whatever nature and kind, as well smoking is expressly prohibited on the minor/ward as mandated by law and city ordinances. Violation thereof shall be immediately escorted out of the establishment and shall be banned from entering the same in case of habitual recidivism.

The management of ______________________________________, its partner, affiliates, agent, managers and employees thereof shall in no case be liable, and shall forever be discharge from its responsibility arising from fault and/or negligence of the parent/legal guardian for the unlawful consumption of alcoholic beverages and smoking by a minor/ward.

It is further understood that in case of accidents and injuries that may arise involving a minor, the above-named parent/legal guardian, knowing fully well the risks involved hereof, hereby agree to assume those risks and to release and to hold harmless the management and all of the employees and/or agents acting for and in its behalf, who might otherwise be liable to me (or my heirs or assigns) for damages. It is further understood and agreed that this waiver, release and assumption of risk is to be binding on my heirs and assigns.
