Feeling tired or weak; Pale skin, lips, and nails; Dizziness; Shortness of breath; Rapid heartbeat; Trouble concentrating

Monitoring of vital signs, capillary refill, color of skin, & mucous membranes. Raising the head position in bed. Check and document the presence of pain. Observation of a delay in verbal response, confusion, or restlessness. Observing and documenting the existence of the cold. Maintain the ambient temperature to keep warm the body needs. Provide oxygen as needed as prescribed.

Adequate tissue perfusion oygen saturation between 95-100%. Client will be alert to time, place, and self with in four days.

Client's tissue are perfusing adequately at 98% oxygen saturation. Client is aware of self, time, and place.

Client will demonstrate the important of diet and exercise to prevent anemia at time of discharged. Client stated he has been drinking strong black coffee in the morning without breakfast and, at times, does not eat till 16-18 hours later due to work demands. Client has black stool.

Nurse teach client the need to eat breakfast then coffee to prevent ulcer. Coffee is acidic and needs food to obsorb or with milk. Dr. prescribed omopril and ferrous sulfate to help with preulcer.

Client was very cooperative and understands the important of good diet and exercise. Client is more active and bowel elimination is normal. Client has been eating three meals with coffee after breakfast. Client also added milk in coffee. At time of discharge client states he will continue this diet and exercise.
