6.1 Bank will collect the annual premium and take the consent for subsequent renewal premium on a specified format.

6.2 Customer will sign the Good Health declaration at the time of joining the scheme. There would no other documentation signed by customer at the time of joining the scheme.

6.3 Bank will keep the Health Declaration duly signed by the customer with other account opening form and documents and produce the same to . Life at the time of Claim. GHD as attached in annexure A.

6.4 Bank will get the standing instruction signed by the customer in case where customer wishes to renew the policy on every year basis.

6.5 The member wise data in a consolidated format shall be shared with . Life on daily basis.

6.6 . Life to extend the coverage to the account holders on Bank to share the detail captured in the system with . Life.

Bank Official will fill in the details in a portal provided by . Life. System will validate the entries and the Certificate of Insurance (COI) will be generated for Bank and depositors record.

6.7 Bank Official will obtain the Good Health Declaration (GHD) signed by depositor and also verify and keep the duly signed GHD in record with account opening forms and other dockets. GHD to be furnished to . Life at the time of claim arises.

6.8 . Life will reconcile the entry with premium deposit and confirm the bank on daily basis. Any discrepancies if found will be marked to Bank authorized Official for rectification.

6.9 The data of members, who have foreclosed account in a particular month and hence cease to be a member of the group, is to be shared with . Life. The coverage will continue till the policy renewal date and renewal will not be processed.
