1. Patient is a female of African descent in her mid-30s.

Chief Complaint: pain in her stomach. She has a history of kidney stones and is scheduled for surgery in three weeks to remove the stone from the renal pelvis.

Diagnosis: Urine dipstick test is negative. Inflammatory damage to kidney tissue becomes susceptible to bacterial infection.

Prescription: Ciprofloxacin 500 bid for prophylaxis. Antispasmolytic drug Buscopan was prescribed to reduce hyperactivity of her stomach. A proton pump inhibitor, omeprazole 20mg was prescribed to alleviate the pain from reflux from the previous antibiotic.

2. Patient is a female of African descent in her 30s.

Chief Complaint: she is having a reaction on her chest. She thinks it may be due to the blood thinner (heparin) which she started two days ago. Patient has been diagnosed recently with breast cancer and breast cancer runs in her family.

Diagnosis: Patient is susceptible to lung emboli and thrombosis, which is why she has been prescribed a blood thinner. It is uncertain whether or not the heparin is causing her allergic reaction. Chemotherapy began two days ago. Gave the patient two shots of heparin and watched to see if there was an allergic reaction. No allergic reaction occurred, which means the heparin is unlikely to be the cause of her allergic reaction.

Prescription: Advantan as an antihistamine for itching/allergic reaction. L-ergothionine an antioxidant is given for treatment of the adverse effects of chemotherapy.

3. Patient is a 13-year-old female of African descent.

Chief Complaint: Pain in her vaginal area and dysuria.

Diagnosis: upon examination the urethra is inflamed and there are white pustules in the vaginal vestibule. It is most likely to be a gonorrhea infection.

Prescription: Ciprofloxacin 250mg bid and declofinactin (NSAID) for pain.

4. Patient is a female in her 50s of African descent.

Chief Complaint: She has been coughing for three weeks and feels like she cannot breathe....
