Benefits that will be offered with the company plan:

There are five categories different benefits can be separated into.

1. Compensation:

* Base salary

2. Insurance:

* Medical insurance

For individual the annual deductible is $300 and for a family is $600. You will have a co-pay of $20 for office visits and consultations. There is an 80/20 once your deductible has been met. Prescription drugs are covered at 70%. Medical Premiums are $150.00 for singles and $300.00 for a family.

* Dental insurance

Dental has a $50 deductible for basic dental care and $150 for Major Services. No waiting period for major services. Basic dental work such as cleanings and X-rays will be paid at 100%. Major Oral work such as Oral Surgery will be paid at 50% and Dental Repair Services will be paid at 75%. Dental premiums are $46.00 a month for singles, and $91.00 for a family

* Vision/Eye care insurance

You can get an eye exam every year and paid at 100% once you have paid $10 co-pay. Classes and contacts will be paid at 100% every two years. Vision premiums are $10.50 for singles and $25.50 for a family

* Life insurance (Available)

Life insurance is paid by your basic annual earnings and is free to the employee.

* Disability insurance (Available)

Short term is available up to 26 weeks and long term disability is available after the short term disability has expired. Long term is paid at 50% of your pay.

3. Time benefits:

* Vacation days-For a Management employee, once you work 6 months, you receive one week and after 9 months you receive another week and after 12 months you will receive another week. All others, Once you work 6 months you will receive one week and after 12 months you will receive another week

* Paid Holidays- There are 7 paid holidays (New Year’s Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving and Christmas)

* Sick days- Sick days are given out January 1 of each year. Management will receive 80...
