The increase in the ageing population within the present social and economic situation must take into consideration the acute and chronic healthcare needs as chronic complications are almost significant (Department of Health {DoH}, 2001). In this assignment, the author is going to explore on Type 2 diabetes as it is one of the common health conditions in the older community. There will also be an exploring of the care of patients with Type 2 diabetes in the particular reference in between acute care services and care in the community so as to meet the needs of this group of patients. Type 2 diabetes chronically can lead to acute episodes which require urgent intervention and treatment. Also in this assignment, the author will focus in the nursing holistic care approach to patient with this condition (Type 2 diabetes), including the management, prevention, and assessments to monitor blood sugar levels, diet, and exercises.

The DoH (2001) National Service Framework (NSF) for Older People has made some vital targets and improved care of the Type 2 Diabetes group of patient in society. The Framework’s main concern is aimed on the person-cantered care. According to DoH (2002) literature and policy guidelines on diabetes, which can be persistent causing some physical problems, it is revealed that this condition has got a negative impact to individual’s normal life functions including their health and well-being. There are also socioeconomic effects associated with Type 2 diabetes (DOH, 2002). Hilton and Digner (2006) states that there are about 1.4 million people in the U.K who suffer from diabetes. The guidance provided by the National Service Framework for Diabetes (2002) is on looking after managing the condition with an outline to proficient treatment with best care management including health promotion and avoidance of acute illness caused by the condition ( DOH, 2002).
