APPENDIX “A”

THIS DOCUMENT WILL BE SIGNED BY EVERY STUDENT IN EACH CLASS DURING THE TWO WEEK OF THE CLASSES IN EACH SEMESTER. FAILURE TO DO SO WILL RESULT IN AN ADMINISTRATIVE DROP FROM THE CLASS, AND/OR A WITHHOLDING OF GRADES.
