Obtain Foster Parent Licensure
Overview: The purpose of this project is to obtain foster parent licensing for my home.
Objectives: My goal is to complete necessary training, physical and psychological evaluations, background checks, and home study. My goal is to do this within a budget of $1100.00 and to complete by June 30, 2005.
General Approach: My husband and I will attend foster parenting classes for six weeks and obtain CPR/First Aid certification. My husband, I, and my son will obtain evaluations from both our family physician and a behavior health specialist for physical and mental clearance. My husband, I, and my son will obtain TB skin testing and immunizations from the local health department. My husband, I, and my son will fill out and submit central registry, local, state, and federal background checks and submit to the appropriate agencies. My husband and I will complete licensing applications and attend interviews with the DHS agency. My husband and I will prepare the home for licensing home study to include safety of all hazards, fire plan, and room preparation for future foster children. Key resources include the labor of myself, husband, and son, money to pay for training, evaluations, home safety improvements and furniture.
Evaluation Method: Key performance metric will be the awarding of foster parent licensing within the budget and time frame.
