Analyse the information, identify the potential causation factors.

Using the information in your learning guide and textbook write a report to the relevant industry/organisation’s stakeholders, ensure all legislative requirements are addressed in your report.

Gather & organise the injury data and attach as an appendix to your report together with any other information you deem relevant.

Include in your report tables and graphs showing the area of injury, severity and average time lost. Also include a distribution list identifying all relevant stakeholders and interested parties, explain why

you think this information is relevant or important to them.

Introduction

The purpose of this report is to outline the main five (5) causes of injury within our organization and to identify the causation factors. The objectives are to highlight the areas of concern, and to make recommendations as to what corrective actions may be undertaken to resolve the issues. Although investigations into the individual issues have been undertaken previously, this is the first report to include all the main areas of concern on one single report.

As time was limited to conduct research for this report only informal discussions and general observations have taken place limiting the findings of the report.

The method of data collection used was analysis of incident reports, as well as the analysis of compensation claims...
