At this time, I am requesting that you call our firm to schedule a meeting as there are some follow up questions and information that we will need in order to proceed with our investigation of your case. A copy of any medical records, bills or invoices (Paxton ICU)

A copy of any medical records, bills, invoices from Metropolitan Memorial Hospital

A copy of any medical records, bills, invoices from psychiatric hospital (Kansas City)

A copy of any information you may have received from the ambulance company in Bloomington

A copy of the release form you may have signed

Verification of your driver’s license and insurance information of your motor vehicle

Verification of your employment and how long you were employed

Verification of your school status at the time of the accident

Please schedule the meeting as soon as your schedule allows. Your continued cooperation in this matter will expedite this process more efficiently and prevent delays. If you have any other documentation in your possession that you feel would support your case against the defendants, we encourage you to bring them as well.
