A copy of any information you may have received from the ambulance company in Bloomington

A copy of the release form you may have signed

Verification of your driver’s license and insurance information of your motor vehicle

Verification of your employment and how long you were employed

Verification of your school status at the time of the accident

Please schedule the meeting as soon as your schedule allows. Your continued cooperation in this matter will expedite this process more efficiently and prevent delays.
