How would creating a new position between the CEO and the location managers help the business to grow?

It seems like Lei is taking up a lot of time helping his managers at the locations, and not enough time on his business as a whole. If he was to create a new position Lei can make all the major decisions for the company and delegate more responsibilities to the location managers.

2. Is promoting an existing manager the best option to fill this position? If not, what is an alternative source to fill the position?

When looking for a new position I believe that one of the existing managers would be a good choice, because they already know the mission and would be the best to oversee the organization as a whole. The managers already have good management skills because of their knowledge and experience already in the company. Another alternative is to go outside the company and get someone experienced in being a CEO because they can also bring their strengths they may have from another company.They may be able to add insight and enhance Sandwich Blitz. Both are good options I believe it is about how much do they want to train the first option would be an easier transition.

3. Who within the company should make these decisions? The CEO 's should make the ultimate decision to add on another position

The decision should ultimately be Dalman and Lei, since the are the owners so they have the ultimate authority in the company. They know exactly what they want for their business son who else to make the decision.

4. List the levels of authority (management) that Sandwich Blitz, Inc. would have if the new position is created.

It would start with the CEO, then the top manager, then the location managers and finally the associates. as it goes normally the top management are able to make a little more decisions then the lower management but the CEO is able to make the ultimate decisions.
