Dear Mrs. Johnson/Team62,

On September 10th, 2010, I was severely injured in a car accident by your insurer Vicki L O’ Neal. Your insurer is responsible for this accident due to her failure to abide by N.C traffic safety laws. Eye witness, (Detective Scott Tanner) with the Pitt County Sheriff’s Office, Greenville N.C, 27835 which was off duty, witnessed your insurer being at fault, see exhibit _(A)________.

As a result of the collision’s impact, I sustained a lumbar contusion, which was diagnosed by attending Physician Karen Kinney at Pitt County Memorial Hospital ER Department. Additionally I was prescribed several pain medications and was advised by my discharge papers to follow up with a family physician, this information was faxed to you in 2010.

While I was in progress of consulting with a family Physician for an office appointment due to pain, I was incarcerated for a probation violation on 9/24/2010, see exhibit _(B)_______, however in the initial phase of incarceration I was taking my prescribed treatment medication from the ER.

On 10/21/2010, I was seen by a Department of Correction Physician and was diagnosed with lumbar strain (acute) which progressed to lumbar strain (chronic) which is the underlying condition from result of the 9/10/2010 accident, see exhibit _(C)_pg.11__.

The treatment for the conditions consisted of me taking a host of pain medications, walking with the assistance of a (cane) for one year, see exhibit _(C)_pg.12______, and (PT) physical therapy, which was recommend for one year, see exhibit _(C)_pg.11______.
