1. Go to file, then select new. Change the value of width and height into 8 and 4 inches then ok.

2. Select the gradient tool on the tool bar.

3. Drag the cursor inside the working area.

4. Select type tool. Type your name at the upper-left side of the image.

5. Under the name type your course, year and section.

6. Select rectangle tool. Make 4 rectangles.

7. Select type tool, on the top of each rectangles, rename it with Attendance, Activities, Quizzes and Midterm/Final Exam.

8. Put your cursor on the rectangle tool then right click. Select line tool.

9. Make boxes inside the rectangles using the line tool.

10. Go to file, select open then select your picture.

11. Press ctrl+t to resize the picture then drag it to the Untitled-1. Replace it on the upper-right side of the image.

12. Then save it. Go to file, select save as, select the drop down arrow in the format then select jpeg then save.
