Tubo-ovarian abscess (TOA) is a complication formed from the disease pelvic inflammatory disease (PID). When a woman contracts the disease, a pocket of pus, which is a tubo-ovarian abscess, may form in the fallopian tube and ovary during the infection. Women with multiple sexual partners are at high risk of the infection.

The infection, pelvic inflammatory disease, causes infections within the upper and lower tract of the genital. PID is classified as a polymicrobial disease, meaning the cause from several types of microorganisms. Sexually transmitted diseases, such as Neisseria gonorrhoeae and Chlamydia trachomatis, are implicated in a woman developing the disease, but rarely are recovered from an abscess. A woman, who contracts PID, may show no signs or symptoms, which will make it difficult to diagnose. Complications associated with a tubo-ovarian abscess include infertility, ectopic pregnancy, chronic pelvic pain, pelvic thrombophlebitis, and ovarian vein thrombosis.

Past treatments for a tubo-ovarian abscess required bilateral oophorectomy (removal of both ovaries) and hysterectomy(removal of the uterus). Now, treatments range from intravenous antibiotics for at least 24hours with Cefotetan 2g IV every 12 hours or Cefoxitin 2g IV every 6 hours, to Doxycycline 100mg orally or IV every 12 hours. After the parenteral treatment is completed, it is recommended by the CDC that clindamycin or metronidazole be used with doxycycline for a total of 14 days treatment.
